Combined use of Miniscrews and Clear Appliances for the Treatment of Bialveolar Protrusion without Conventional Brackets

Objective: The increasing demand for esthetic orthodontic treatment has led to the development of either tooth-colored or even ‘invisible’ appliances. Although Invisalign® has several advantages over other conventional braces, it has some limitations when applied to extraction treatment. This case report presents the esthetic extraction treatment of a bialveolar protrusion without using conventional brackets.
Methods: The treatment was completed in two steps. The first step involved segmental retraction of anterior teeth using miniscrews and an anterior lingual splint with a clear lever-arm. The second step involved alignment and finishing using sequential removable clear aligners.
Results & Conclusions: After treatment, the protrusion of the upper and lower lip, which was the patient’s chief complaint, was properly corrected and the strain of mentalis muscle on lip closure was relieved. This poster presentation shows the esthetic extraction treatment of mild Class II bialveolar protrusion without the use of conventional brackets. The treatment was completed in two steps, which consisted of segmental retraction followed by alignment and finishing. There are some advantages of this method: 1) it can satisfy the patient’s esthetic demand during treatment due to the invisibility of the appliances; 2) the inclination of anterior teeth can be controlled efficiently by the application of lever-arm; 3) maximum retraction is possible without anchor loss with the aid of miniscrew; 4) it is remarkably economic compared with Invisalign®.
