Management of Asymmetric Class III Dento-Facial Deformities
Introduction: The asymmetric class III dentofacial deformity has been referred to by a variety of terms including deviate mandibular prognathism, mandibular laterognathism, condylar hyperplasia and osteochondroma of the mandibular condyle. This presentation would include case reports of different types of asymmetrical Class III dentofacial deformities, their diagnosis and treatment modalities adopted essentially and jointly by the orthodontist and maxillofacial surgeon.
Material and Method: Twenty adult patients identified as asymmetric Class III dentofacial deformity were treated. Presurgical orthodontic treatment was done to make the occlusal asymmetry equal in magnitude with that of the skeleton to both maximize correction of the facial esthetics and the stability of results and avoid unnecessary surgery in the maxilla. Depending on the location and magnitude of deformity, viz. chin,mandible,chin and mandible,mandible and maxilla, surgical procedures like straightening genioplasty, bilateral sagittal split osteotomy, vertical sub signmoid osteotomy, Le Forte I osteotomy,were performed.Post surgical orthodontics was done for finishing and detailing of the occlusion.
Results: Out of 20 cases of asymmetric deformities, 18 cases were primarily of mandible and 2 cases of both mandible and maxilla.
Conclusion: The optimal esthetic results with stability in these cases were achieved by anteroposterior reduction with rotation of the mandible along with planned pre and post surgical Orthodontics.
